Club Account Check Request

Request a check or petty cash from your club account with this form. If you are asking for funds from a Student Programming Fund that has been allocated to your organization, please use the Student Government Check Request.
Make sure you review the Club Policies and Procedures document before proceeding.

Student Government Check Request

Mainly used for Student Government internal accounts. Clubs and Organizations will need to use this form if they are being reimbursed through Student Programming Funds.

Performance Agreement

A Performance Agreement is required anytime a speaker or performer is brought on to campus. The PA serves as the contract between the a campus group and the speaker/performer they are presenting. This form enables you to request a Performance Agreement for your event, it DOES NOT create the agreement. Please follow the directions after you submit the form in order to ensure that your Perfomance Agreement is fully processed.
