Club Account Check Request
Request
a check or petty cash from your club account with this form. If you are
asking for funds from a Student Programming Fund that has been
allocated to your organization, please use the Student Government Check Request.
Make sure you review the Club Policies and Procedures document before proceeding.
Student Government Check Request
Mainly used for Student Government internal accounts. Clubs and Organizations will need to use this form if they are being reimbursed through Student Programming Funds.
Performance Agreement
UPDATE:
FOR INTERNAL USE ONLY for ASUCI performances.
Campus orgs DO NOT NEED to use performance agreements. Payments to speakers and performers should be done by CHECK REQUEST ONLY.
If performance is to be paid using SPFB funding, use a Student Government check request.
