This is a REQUEST form, not the actual performance agreement! Read the procedure below.

To fill out this Performance Request Form:
•  Log in with your UCInetID and fill out ALL the fields.

•  Check the request for any mistakes. If there are any, click on the "Make Changes" button.

•  If there are no mistakes, PRINT THE PAGE FIRST.  After printing the page, click on the "Create Request" button.  THE REQUEST MUST BE SIGNED AND TURNED IN TO THE ASUCI OFFICE - B107 STUDENT CENTER (the signature must be of someone who is on file with financial authority for your club account with the ASUCI business office).

If THIS COPY of the form is not turned in with a signature, the request will not be created. Remember, this is a request form, not the actual agreement!
